When is the best time to begin Orthodontic Treatment?
Most countries recommend first orthodontic consultation for all the children by the age of seven. Luckily, an early orthodontic screening examination allows orthodontists to best determine the optimum time before the problems get too difficult. This does not mean that all children need treatment at this age, but it does allow the assessment of each individual case to determine who might benefit from early intervention. A. There are fixed appliances that include:-

Then the arch wires are passed through the brackets and attached to the bands and when tightened the arch wire puts tension on the teeth, gradually moving them to their proper position. Today's braces are available in various shapes, sizes and light-weight to suit the style statement of kids and adults alike.


Special fixed appliances:  For the thumb sucking or tongue thrusting kids (as also adults) these appliances are their nightmares.  These are attached to the teeth by bands and are very uncomfortable during meals. These successfully act in breaking the habit, but are only as a last resort.


Fixed space maintainers: There are times when a baby tooth is lost prematurely. In order to keep the space open until the permanent tooth erupts, a space maintainer is placed. A band is attached to the tooth next to the empty space, and a wire is extended to the tooth on the other side of the space.


B. Then there are removable appliances that include:-
Removable space maintainers:  These are made of acrylic base and fit over the jaw and serve the same function as fixed space maintainers. But to keep the space open between the specified teeth, there are plastic or wire branches.


Jaw repositioning appliances:  These devices are also called by other name as “splints” and worn on either the upper or lower jaw. These devices help in moving the jaw to close in a more favorable position. These are often used for temporomandibular joint disorders (TMJ).


Lip and cheek bumpers: In situations when lip and cheek muscles are exerting pressure on teeth, these bumpers are so designed to maintain the distance and relieve that pressure.


Palatal expander: This plastic plate is fixed by screws over the roof of the mouth. Designed to apply outward pressure on the arch of the upper jaw, the device help it open lengthwise and widen the palatal area.


Removable retainers: After the completion of orthodontic treatment, these devices are worn on the roof of the mouth. These devices prevent shifting of the teeth to their previous position. They can also be modified and used to prevent thumb sucking.


Headgear: Headgear is a device in which a strap is placed around the back of the head and attached to a metal wire in front, or face bow. Its purpose is to stop the upper jaw from growing, hence preventing or correcting an overbite. Headgear slows the growth of the upper jaw, and holds the back teeth where they are while the front teeth are pulled back.


Aligners: These are relatively new alternative to traditional braces for adults. A set or series of aligners are used to move teeth in the same way that fixed appliances work. There are no metal wires and brackets. Since these are made of special plastic material, these aligners (or trays) are virtually invisible and are removed for eating, brushing and flossing.


What is Combined Surgical Orthodontics?
Surgical orthodontics is also known as orthognathic surgery. No matter how good the orthodontist is, he or she cannot move teeth anywhere the bone will not follow. An orthodontist repositions teeth, while oral and maxillofacial surgeon uses orthognathic surgery to reposition one or both jaws. Just as "orthodontics" means "straight teeth", "orthognathic" means "straight jaws". One of the largest advantages of surgery combined with orthodontic treatment is the marked improvement most patients have in their facial appearance after treatment. In fact, because moving the jaws also moves the teeth, orthognathic surgery is usually performed in conjunction with orthodontics so that the teeth are in proper position after surgery. It is more often performed in severe underbite cases. As a result, those who could not chew their food can now eat without difficulty and the patients who could not close their lips around their front teeth now can after the combined treatment of surgical orthodontics. In many cases an enhancement in the appearance can also be seen.
